Alright summary on transition so we went through the clinical summary before in the clinical summary was for patients to get their information ID system the summary transition is actually for providers so this is a little bit different but you’re gonna be using similar screens so this one the eligible provider who has the transition is going to, to its is doing the transition to another setting of care uhm is going to to and pull this out a system so they can give it to that other provider so you can see the clinical encounter screen is the same one that we use for the medication reconciliation you get to use it for the a the summary on transition as well, basically what you’re gonna do is instead of inbound you gonna check outbound and now be outbound of your system so encounter should be designated as outbound and the options inbound or outbound or neither are available and neither is just if you don’t check any the boxes this is the next screen you’re gonna get so this is the clinical summary report we’ve already seen it once but because we’re doing this for an actual provider, ahm what you’re gonna get a little bit different output, so you can check all the things that you wanna pull under transitional of care and you can put in the dates for it then you can actually pick which kinda output that you’re gonna have for that so, you can do the printing or an electronic generation, the electronic is gonna be the CCDC32 2.5 I almost said c3po that was that will how nerd I was I guess I just did ahm when prompted check summary of care ahm no record provider for care coordination and then here’s the, the required content so diagnostic lab results, problem list, medication list and medication allergy so all those things have to be on that when you’re sending it to another provider here’s the statistic behind that you have to meet so it’s looking for more than 50 percent other transitions of care and referrals have either one is printed or electronically sent.

Here’s the denominator so there is at least one encounter in that reporting period and has to be by the provider and the encounter must be marked outbound and then here are the number of clinical encounters in the denominator with a summary of care provider for the care coordination flags mark so basically however many you have of those has to be over 50%..
