In 2002 Andranik Ovassapian, Allan Klock, Gain Randel and others wrote a landmark study on difficult intubation. They showed that every unanticipated difficult intubation patient at their institution had lingual tonsil hyperplasia. The study was not controlled and we don't know how sensitive or specific the finding is. This video describes one such patient I took care of recently.
