Angioplasty and stenting during a massive heart attack! Steps in a classic Primary angioplasty video! Dr Binoy John

This is a classic case of what happens during a heart attack. Heart attacks are usually caused by a sudden 100% block caused by clots occuring in one of the arteries which supply blood to the heart. Blood clots are the culprit which cause heart attacks in majority of the patients. 

This is a young man 38 years old with no risk factors who presented to the emergency with severe chest pain. ECG revealed that he was having a heart attack involving the Right coronary artery. He developed arrhythmias and brief Cardiac arrest and he was immediately shocked and ventilated.

He was immediately taken up for Primary angioplasty, which means emergency immediate angioplasty usually with stenting to quickly open the blocked artery and restore back blood supply to the heart muscle. This is the best treatment in contrast to medical management with injections to dilute the clot which may or may not be successful and has no guarantee.

The wire was passed across the block and the angiography revealed large volumes of clot in the entire course. Clots were sucked out using a suction catheter. (The extracted clots and before angioplasty and after angioplasty images with complete flow established can be seen at the end of the video) After sucking out the clots a stent was placed to completely open up the block and re establish complete flow in the artery. 

The entire procedure took 2 hours.
The patient was subsequently extubated from ventilator and was shifted to the ward on Day 4 and discharged completely fine on Day 5 with normal Pumping function of the heart by God's grace.
