This case is one that Dr. Caridi sees from guys all across the globe. Our patient has worked hard to lose weight but the gynecomastia can't be removed under a bench press. We removed it with great results right on the table and his journey gets better and better as we keep up with his recovery process.

The healing process is what Dr. Caridi is most concerned with because once a patient goes home, Dr. C can no longer monitor them. Managing the healing process largely affects the outcome of the results, so it is stressed to every patient who comes to the Austin Gynecomastia Center.

This is an exciting segue to mention that Dr. Caridi has been working on a paper over the past few years, that has been accepted in the Plastic and Reconstructive Surgery Global Open! This paper fully encapsulates and describes what Dr. C does – this concept of total tissue removal with layered flap closure.

The paper is a synopsis of analyzing hundreds of gynecomastia cases that Dr. Caridi performed from 2018-2019.

The thinking amongst many plastic surgeons has historically been that you need to leave some gland behind in order to prevent creating a divot or depression.

Gynecomastia tissue is different. It's well-circumscribed and is in a circular area making it more difficult to get out than with just suction alone- this is why the Vaser system does such a great job. Vaser is an acronym for a system that uses ultrasonic energy to cavitate the fat (think about this like melting the fat). The Vaser does not remove all the tissue, but it does help to shrink the tissue in the chest area down while separating the gyno tissue from the surrounding normal tissue.

One of the main questions addressed by this paper was: Can you remove all the tissue and NOT have any issues related to depressions or divots? After following over 560 cases, the answer to that question is a resounding YES. Dr. Caridi didn’t have a single request from patients to address concavities or depressions post-operatively when removing all the tissue.

What was learned through this paper, is the primary concern when dealing with a gynecomastia procedure has to do with managing scar tissue. This revelation is why Dr. Caridi focuses so intently on the healing process once a patient goes home.

Dr. Caridi’s paper also showed that complication rates with his technique are comparable with previously published techniques (these rates are already very low). Interestingly, Dr. Caridi has recently noticed his patients have fewer hematomas and less scar tissue. This is likely directly related to the vast amount of time focused on post-op management.

Helping smooth out the result between this “high/low” phenomena on the upper and lower part of the chest is done in part with an internal flap closure, explained in the paper, to leave the chest looking very natural.
