For this resident case there is an extensive posterior sub-capsular cataract which is causing poor vision, decreased acuity, and tremendous amount of glare with oncoming lights (headlights during night driving, sunset / sunrise, etc). The question for this case, is with this younger patient (55 years old) who is currently a -2 myope, what would be your recommended IOL and refractive target? Stick with -2? Go for plano / emmetropia with a monofocal IOL? EDOF IOL? Trifocal IOL? Even if the trifocal IOL induces some nighttime dysphotopsias/glare/halo, it has to be much better night vision than the patient currently has. Think about it and leave a comment below.
