What is Rehab Pilates?
Rehab Pilates is a therapeutic exercise tool used to address biomechanical compensatory patterns and dysfunction. Using Rehab Pilates principles, we can break down basic movements and functions of the body and use those same principles to build you back up. We start with very simple movements and concepts and we practice doing them well. From a rehab perspective, if you’re not doing it well, you’re not doing it. Once you’re doing it well, we gradually make it harder or more complicated.
Here’s an example I see in the clinic almost daily; an unstable pelvis or SI joint, that has “gone out” of alignment and is leading to lower back pain or sciatica. There are lots of reasons this can happen, but for this example I’m going to keep it simple. The right side of this person’s pelvis is stuck in a posterior rotation. I could put it back in and send them on their way, but I know they’ll be back soon with the same problem, because the driver of the dysfunction wasn’t addressed. So I look at a few things, including having them lay face down and lift their right leg off the table. I can immediately see that they like to use their hamstring rather than their glute max to extend their right hip. When I ask the patient to tell me where they feel the effort, they say the hamstring, which they may not have noticed unless I asked. This is a reverse firing pattern, it should be the other way around. The hamstring is working so hard that it’s become tight and short, pulling on the ischial tuberosity which pulls the right side of the pelvis into a posterior rotation. So this person needs some hamstring release work (stretch or self massage), followed by activation of their glute max to get it turning on. They need to practice this often to retrain the nervous system to stop using the hamstring primarily. Correcting this can be the difference between a healthy hip joint and hip replacement later in life. An overactive hamstring can drive the head of the femur anterior in the socket, which causes it to rub on the acetabulum of the pelvis. Over time, this contributes to OA of the hip.

A common misconception about rehab exercise is that the target muscle is weak, when in fact it’s usually a bad neural connection to that muscle. It’s like your brain and that muscle are estranged old friends who need to reconnect. Pilates can help you make that connection.
