Want a good reason to hate big pharma? Premarin is one of the most prescribed hormone replacement drugs in the world, but would you believe me if I told you it comes from horse pee, and at the cost of hundreds of thousands slaughtered foals?

Behind this “normal” medication is a hidden industry of cruelty.

On PMU (pregnant mare urine) farms, thousands of horses are kept in tiny stalls, unable to move freely, never free to roam or graze. They are repeatedly impregnated so their concentrate urine can be collected through heavy catheters and restrictive devices strapped to their bodies 24/7. Many mares suffer painful sores, infections, dehydration, and exhaustion. Once their usefulness ends, many are sent to slaughter. Their foals are considered “byproducts” and are sold for meat or disposed of.

The shocking part is that this cruelty has been happening for decades — yet many people don’t realize their prescriptions fuel it. Beyond the animal suffering, major studies like the Women’s Health Initiative have also linked long-term use of Premarin and related drugs to serious health risks including heart attack, stroke, blood clots, and breast cancer.

Safer, synthetic, and plant-based alternatives exist — but Premarin continues to dominate the market because of profit and tradition. We need to do better about proactively looking to alternatives to this drug because behind every prescription is a foal or mare who suffers or dies.
