What is an AMH? It is an Anti Mullerian Hormone. It is basically a blood test. At the initial checkup, we evaluate the couple to basically do a scan along with hormonal blood test and AMH is one of them. It is a marker of their ovarian reserve. Ovarian reserve is nothing but where do you stand on a fertility scale. As woman you should know that the age is very important to us when compared to men. Men are very lucky and they can have their sperm production once in 3 months, which we do not have. So whatever number of eggs we are using these we are using from the time of our birth. These are the number of eggs which are there in our ovaries and we are utilizing it. Every month there is an egg release. In order for one good egg to get released there are thousands of other eggs that gets wasted in this process. So our age is very important and as we age, after 35, our pregnancy chances goes on decreasing and that indirectly shows that our number of eggs are coming down. So what are the reason for AMH being low? Normally the AMH value is in between 2.2 to 4 ngm/ml. Anything less than that is considered as low AMH or low ovarian reserve. The reason being, a is mentioned the age, is there an exposure to radiotherapy, is there any cancer survivor you have underwent chemotherapy, any operations in your ovary or any ovarian cyst that is taken out, any pelvic infections, any autoimmune disorders all this can contribute to a low AMH. In females it causes low AMH. Smoking is the main prime factor and it has got effects on male and female fertility. Smoking causes effects of low AMH. So to drastically reduces the eggs let behind and in makes it reduces the sperm motility. I will be discussing about the importance of low AMH and fertility. So now you do understand that low AMH do not the reason for being infertile . low AMH just implies that your ovarian reserve is less and you might require more dosage of injections or little more treatment in terms of ovulation induction or IUI or IVF compared to other normal AMH levels. So what are the treatment options available in a woman with low AMH. The first thing is what is the age of the wife and how long you have been married. So the married life is 1 or 2 years and not taken any support of fertility treatment, then start with the basic ovarian stimulation or ovulation induction, which gives us the chance of 10% pregnancy in each month after 1 or 2 cycles of ovulation induction, then we can try for intrauterine insemination or artificial insemination, which gives around 20 to 25% chances of pregnancy. If this fails, then we can try for IVF. IVF gives about 50 to 60% chance of pregnancy and in the can you see there are only few number of follicles, then the last resort is to go for a donor one. That is a doctor occult IVF. So after 1 or 2 cycle so of IUI, then pregnancy doesn’t happen, then we have the IVF. In IVF when the number of eggs are less, there is something called as the egg pooling wherein we do couple of cycles of IVF like if you have only 1 follicle, you sat giving the infestation for that follicle to gir wan we do an egg pick u with the husband’s sperm t get an embryo and freeze it. One more cycle of an IVF. In this way we can atleast do atleast till we get 2 to 3 embryos and transfer the embryos that gives around 50 to 60% chances of pregnancy. Inspite of that if there is no pregnancy, then the last resort is to go for a donor oocyte, For that the donor will come, give egg pickup from the donor, which with the husband’s sperm from the recipient and transfer the embryos into the recipients uterus. So this gives us 70 to 75% chances of pregnancy in all the chances. In Janani Fertility we have been getting around 70 to 75% chances of pregnancy in all the cycles.
