Placing implants immediately after extraction in a mandibular first molar socket is always challenging. The main issue being the slippage of the drill into the mesial or distal area due to presence of a thin furcation zone. In such cases it is prudent to finish preparing the entire osteotomy through the roots first, followed by removal of the roots and then installing the implants. Achieving primary stability is of paramount importance as is the necessity of doing the procedure flapless. The video demonstrates this step wise.
Smith and Tarnow have classified these sockets into three types based on the amount of available interradicular bone. This case belongs to Type A socket classification.
