Rhinoplasty – A Common Challenge in South Indian Cases - **Educational Purpose**

One of the key challenges we often encounter in South Indian rhinoplasty-whether surgical or non-surgical-is achieving a sharper nasal tip. Many patients come to us seeking a more defined tip, increased height, and reduced width. While we usually achieve satisfactory results in most areas, refining the nasal tip remains a complex issue.

Post-surgery, the tip tends to swell initially, which is expected. While this swelling typically subsides over time, in about 50% of cases, we notice that the tip can still appear spindle-shaped even after 6 months to a year. Understandably, this leads to dissatisfaction among some patients, prompting us to continuously seek better solutions.

Over time, we’ve developed a structured post-operative protocol that has helped us achieve consistently improved results:

1. Splinting Protocol:
After surgery, it’s crucial for patients to wear a splint continuously for 2 weeks. Once the initial period is over, we recommend wearing the splint at night for the next 2 months. This step is especially important for shaping the tip-not just for bone support. Unlike tapes, which are commonly used, splints provide the necessary structure to help maintain the desired tip contour.

2. Steroid Injections (Triamcinolone):
If mild swelling persists around 6 to 8 weeks post-op, a small dose (3 to 4 mg) of Triamcinolone injected directly into the tip can be helpful. However, we always advise being very conservative with this treatment, as overuse can lead to depigmentation or whitening of the skin in that area.

3. Isotretinoin Therapy:
For selected patients, a low dose of Isotretinoin (10–40 mg) taken for 3 to 6 months has shown positive results. It's essential to monitor liver function and lipid profiles monthly during this treatment. When managed carefully, this therapy can significantly help with tip definition.

By following this three-step approach, we’ve seen a noticeable improvement in the outcomes of our rhinoplasty patients. We hope that sharing our experience will be helpful for young plastic surgeons facing similar challenges. With consistency and care, great results are possible.
