A quick example of the fight from Extreme Cosplay in The Witcher 3: Blood and Wine on Death March! difficulty with enemy upscaling enabled.

I am using a fast attack build with Grandmaster Wolven armour (+ levity enchantment) and fought at level 51 against scaled level 53 enemies.

I recommend you have the following:

Acquired Tolerance (3/3): More potions = easier fight
Superior Northern Wind Bomb
Superior White Raffords Decoction
Superior Tawny Owl: The fight is as night, infinite duration and faster recast times for signs.
Wraith Decoction
If you choose to use the ancient elven sword, socket it with DOT runes like bleeding, fire, poison.
Use Hanged Mans Venom, the enemies are weak to Steel weapons. (I made a mistake in the Video here, the poison damage is not from the Venom it's from the Blade Oil skill I have active from the alchemy tree.)

When the fight starts, dodge backwards, running forwards through the spearmen will end with you being turned into a shish-kabob.

Reapply Quen like a madman when ever it is down, the wraith decoction will save you from the silly mages. Use a dose of White Raffords whenever you fall below 40-50% total health, healing over time from low-life could get you killed.
