Erosive arthritides are a group of inflammatory joint diseases characterized by erosion of articular cartilage and bone, leading to pain, deformity, and functional impairment. Unlike purely degenerative conditions, these involve active inflammation that destroys joint structures.

Major Types
Rheumatoid Arthritis (RA)

Autoimmune disease causing symmetric polyarthritis.

Erosions occur at the joint margins due to pannus formation.

Commonly affects small joints of hands and feet.

Psoriatic Arthritis (PsA)

Associated with psoriasis.

Erosions often accompanied by new bone formation.

Can cause “pencil-in-cup” deformities on X-ray.

Ankylosing Spondylitis (AS)

Primarily affects the spine and sacroiliac joints.

Erosions at entheses (sites where ligaments/tendons attach to bone).

Leads to fusion and reduced mobility.

Reactive Arthritis

Triggered by infection (GI or GU).

Erosions may occur in peripheral joints.

Erosive Osteoarthritis (EOA)

Subset of osteoarthritis with inflammatory features.

Affects interphalangeal joints of the hands.

Shows central erosions with “gull-wing” deformities radiographically.

Clinical Features
Pain, swelling, stiffness.

Progressive joint deformity.

Reduced range of motion.

Radiographic evidence of erosions (marginal or central).

Diagnosis
Clinical exam: Symmetry, distribution, systemic features.

Imaging: X-rays, MRI, ultrasound to detect erosions.

Laboratory tests: Rheumatoid factor, anti-CCP, HLA-B27, inflammatory markers.

Management
Medical therapy: DMARDs (methotrexate, sulfasalazine), biologics (TNF inhibitors, IL-17 inhibitors), NSAIDs.

Physical therapy: Maintain mobility and strength.
