POV:
For Class 1-A to start their hero training, they had to complete a mental health check, in case of any emotional problems that could get in the way.
So, Dr. Shinnou, a psychiatrist, using his quirk, Pastline, looked through everyone's past to see if there would be a cause of emotional distress.
Each student had their own problems, but he was able to safely diagnose them without any harm.
However, when he got to Izuku, because Izuku wasn't relaxed and was fighting his quirk, Dr. Shinnou tried to forcefully look through his past.
Although he was successful for a short period of time, he saw something he wasn't able to understand, making him waver for a short period of time. This cause his quirk to be canceled out by Izuku's quirk, leaving him injured.
