► Pediatric fractures require a different approach than adult fractures, primarily due to the skeletal immaturity of children’s bones and the presence of the growth plate (epiphysis). Understanding the involvement of the growth plate is crucial, as it influences the treatment and prognosis of the fracture. The Salter-Harris Classification is the most commonly used system for categorizing pediatric fractures that involve the growth plate.

Here’s a detailed look at each Salter-Harris fracture type and their clinical significance:

🔹 Salter-Harris Type I: Involves the growth plate only, often occult on imaging, and typically has a good prognosis.
🔹 Salter-Harris Type II: Extends through the growth plate and metaphysis and is the most common type. It generally has a good prognosis.
🔹 Salter-Harris Type III: Involves both the growth plate and epiphysis, entering the joint space. It carries a worse prognosis, potentially leading to premature growth plate fusion and osteoarthritis.
🔹 Salter-Harris Type IV: A more complex fracture extending through the growth plate, epiphysis, and metaphysis, also entering the joint. This type is similar to Type III in prognosis, with potential for growth disturbances.
🔹 Salter-Harris Type V: A crush injury to the growth plate, leading to vascular injury and premature fusion. This type has the worst prognosis among Salter-Harris fractures and is often occult on imaging.

The Salter-Harris Classification is essential in pediatric orthopedics, as it helps to determine the appropriate treatment and anticipate long-term outcomes for young patients.
