...Explosive, made by Dammon. The Orthon Explosive is the only item that can cause infernal burning, and can be crafted from the Devilfoil Masks found within the Underdark. There are 4 total Devilfoil masks that can be found - one at the dig site in the Grymforge, where the cave-in happened. One at the foot of a staircase behind a wall of rubble, and two found in a hidden room with Hellsboars and a Merregon Legionnaire: one is on the legionnaire and the other is on the altar in that room. Once you have at least 1 Devilfoil mask, you’ll need to head to Last Light inn and speak to Dammon. He mentions if you find any infernal material that he can craft something for you and these devilfoil masks are included. After you hand them over, he’ll give you the Orthon Explosive - one for each mask you give him. And what’s great about Infernal Burning is that it cannot be removed by Healing Potions, Wet, or even the Help action. A truly hellish condition indeed.

Next up is a hidden interaction with soul coins! Usually, soul coins are supposed to grant Karlach 1d4 fire damage when she’s raging, or when her hit points drop below 25% - but for some reason, it’s also granting TWO additional instances of 2d4 fire damage when Karlach makes an Unarmed Strike. That means up to 16 additional fire damage per unarmed strike, and 32 if the target is vulnerable to fire damage. This is a pretty amazing amount of damage since soul coins last until long rest, and would be really great when making Karlach an Open Hand Monk.

Next up is Golbraith the Mind Flayer Hunter! A Larian writer sent this message to a fan:
The Glitter Gala, Bloomridge Park, and the Blushing Mermaid form a triangle in the Lower City of Baldur's Gate, and at its centre is Golbraith's Cellar - where the mighty Golbraith Stredivas can be found. A mindflayer hunter of great renown, he has since retired and now spends his days writing - in the hopes of passing his knowledge on to future generations.
This NPC has quite the background story, and even has a gift for you! Galbraith is a character based off a real person - when a fan wrote to Larian thanking them for releasing Early Access so that he and his father could play before his father’s Alzheimer’s progressed to the point he wouldn’t be able to play, Larian decided to show their support to those affected by the disease by adding this fan’s father to the game. And like in the message Larian sent, there is in fact a large hidden area in the cellar, but to get a very rare shield, all you need to do is talk to Golbraith and he’ll give it as a gift.

Next up is the Shield of Scorching Reprisal. This rare shield has a very unique effect on it called Blazing Retaliation. When a melee attack misses the wearer, deal 1d6 fire damage to the attacker. This shield fits right into a radiating orb build which reduces enemies' chance to hit per orb. The shield also grants resistance to fire damage, which is the most common elemental damage type you’ll run into, so this shield is pretty good! It’s also pretty hidden. To find the Shield of Scorching Reprisal, you’ll need to head to Last Light Inn and head North across the bridge. Once across go immediately to the right, and you’ll need one of your party members to pass a perception check in order to interact with some rubble. You’ll then need to pass a strength check to destroy the rubble, under which you’ll find a fallen Flaming Fist’s corpse - and on that corpse is the Shield of Scorching Reprisal.

Next up is the hidden Upper City Sewers. These sewers even contain a Death Knight and are found near the end of the game. Right before the courtyard fight past the high halls, there is actually a secret entrance to the sewers that lets you skip the entire courtyard fight. You can discover this if you use Speak with Dead on the Absolute cultists right before the courtyard and ask them how they got here. These sewers aren’t entirely a cakewalk though - you will encounter goblins, absolute cultists, undead, and Stalker Svigne...
