After finishing residency, Internists have a number of different career options from which to select. Some may choose to practice General Internal medicine while others may wish to specialize in one of the numerous distinct subspecialties of Internal medicine.

Fellowship refers to the additional training that one receives in a sub-specialty following their residency in internal medicine.

Internal medicine subspecialties are governed by two different regulatory boards.. They are the American Board of Internal Medicine that oversees Allopathic Medicine (MD degree) graduates and the American Osteopathic Board of Internal Medicine which oversees Osteopathic medicine in the United States (DO degree).

There are some differences in the "recognized" subspecialties between the two organizations, but some common ones include:
• Cardiovascular Disease
• Hematology and Oncology
• Gastroenterology
• Nephrology
• Infectious Disease
• Endocrinology
• Rheumatology
• Geriatric Medicine
• Sleep Medicine
• Pulmonary Disease
• Sports Medicine
