Perforating dermatosis refers to a group of rare skin disorders characterized by the elimination of altered connective tissue (collagen or elastin) through the skin. This can occur due to abnormal collagen or elastin deposition or defects in connective tissue, leading to skin lesions where altered materials “perforate” or extrude through the epidermis. Four main types fall under this category: acquired perforating dermatosis (APD), elastosis perforans serpiginosa (EPS), reactive perforating collagenosis (RPC), and perforating folliculitis (PF).

The most common of these is acquired perforating dermatosis, which is often seen in individuals with chronic diseases such as diabetes and chronic kidney disease, especially those undergoing dialysis. APD typically presents with itchy, papular lesions, often on the extremities, scalp, or trunk, which may eventually form crusted nodules or plaques. The intense itching associated with these lesions can lead to secondary infection and scarring.

Treatment options for perforating dermatosis are limited and often focus on symptom relief rather than cure. Topical corticosteroids, retinoids, and keratolytics can be used to reduce itching and inflammation. In cases where chronic disease is an underlying cause, managing the primary condition, such as controlling blood sugar in diabetes or optimizing dialysis in kidney disease, can sometimes improve skin symptoms. However, the condition often recurs, and ongoing management is typically needed. Research into newer treatments, including immunomodulatory drugs, is ongoing, although no standard cure has been identified
