Human rights group HRANA reports at least 495 civilian deaths and more than 10,600 arrests within two weeks, though experts warn the real numbers are likely far higher due to severe media restrictions and one of the most extensive internet blackouts in Iran’s history. Medical workers report that many protesters died from direct gunshot wounds to the head and chest, raising serious concerns about intentional lethal targeting.

At the same time, Iran has issued warnings of retaliation if attacked by the United States. President Donald Trump has stated that Iranian officials have sought negotiations, while also confirming he has been briefed on potential military options. Iranian leaders have responded by warning that any US or Israeli attack would make regional military assets and shipping “legitimate targets,” heightening fears of broader regional escalation.

Iranian authorities continue to frame the unrest as foreign-backed sabotage, blaming the United States and Israel while dismissing protesters as “vandals” and “enemies of God.” Critics argue this rhetoric is designed to justify repression and shift attention away from civilian deaths. Meanwhile, three days of state mourning were declared for security personnel killed, with no official acknowledgment of civilian victims.

The protests now extend beyond economic grievances, with crowds openly calling for an end to clerical rule and the removal of Supreme Leader Ayatollah Ali Khamenei. Analysts say the scale and persistence of unrest indicate a deep crisis of legitimacy that repression alone cannot resolve.

As violence intensifies and international pressure grows, Iran faces a critical crossroads: continue down a path of militarized suppression and isolation, or address the political and economic failures driving millions into the streets. For now, the human cost continues to rise, and the outcome remains uncertain.
