I don't think it's that difficult, but you need to get a feel for the tick that you need to be standing next to the boss. You can practice this in the normal mode though: just only stand adjacent to the boss for 1 tick, and be away from the boss for the other 4 ticks. If you're doing it right the boss will never mage you. If you have this down then the boss is very easy

Gear: scythe voidwaker, maybe BGS?

The vent special occurs at a fixed time. In my non-awakened DT2 boss video, Duke Advanced section you can see the attack patterns based on HP. Key thing is that the special happens, you make him target the middle vents (he throws 4 projectile total, might do 5 sometimes but I don't have any video of it). You can also bait 2 projectiles into the same vent (not including the first vent).

If you get acid walk and medusa special at the same time you can always use the back of the room pillars and pray mage. You barely use any food for the first half of the fight so you have plenty to bruteforce it.

Overall thoughts on DT2: great update! Awakened bosses are really challenging which is refreshing from the disappointingly easy ToA. Hope Jagex does stuff like this in the future.
Took me ~290 orbs to clear all the bosses, around 800m GP.
