During Chapter 4, you will have the Objective to "Find the Church Key". You will need to find the Blasphemer's Head for part of this, and that can be found in the Small Cave Shrine which is just north of the lake.

The solution to the Stone Dais in the Small Cave Shrine is shown in this video, which will unlock the gate and allow you to get the Blasphemer's Head key Item.
