Apexification is a dental procedure used to induce a calcified barrier at the tip of a tooth with an incomplete root or open apex. It is typically performed on immature permanent teeth with necrotic pulp, often due to trauma or deep caries. The main objectives are to eliminate intracanal infection by disinfecting the root canal system, induce a calcific barrier at the root apex to allow for proper sealing of the root canal, and restore the original physiologic structures and functions of the pulp-dentin complex. The procedure involves cleaning and shaping the root canal, placing calcium hydroxide or MTA (mineral trioxide aggregate) inside the canal to stimulate calcification and achieve an apical barrier seal, periodically replacing the calcium hydroxide over several months until a calcific barrier forms, and once the barrier is confirmed radiographically, sealing the root canal with a permanent filling material.
