I have show three 50 pence's variants and dates.
i would like to share these errors over time with you, i have many to go though with you.

As you may have seen in coins heat and friction overtime will cause the end product to to have errors.
This could happen in various ways with metal being stretched and pressed.

Metal is excellent in retaining heat, but wears down very quickly.
Errors on coins could through different ways.
Human error is the most common - Not checking quality properly
Trapped coin or object - Causing a different appearance on the coin
Set up on the equipment - Die not fitted properly or coming on to the coins to fast down to hard on a coin causing splits and cracks.
Fitting the wrong Die - Causing the wrong effigy or missing legends or Dates
Re-strike to a coin - Possible misalignment looks to be doubling
Cleaning fluid - Over spill with the polish and causing milking on the coins
Damaged coins - Being dropped and scratched

Looking for different errors can be time consuming but also fun especially when they are worth much more than face value or even more with graded and and confirmed by the mint as they can be unique. This is just another side of collecting numismatics and worth researching.
