This ultrasound clip demonstrates a case of suspected appendiceal mucocele in a 50-year-old patient undergoing ultrasound follow-up for chronic liver disease, with an incidental asymptomatic finding of a mass in the right lower quadrant.
On B-mode imaging, a well-defined, tubular, cystic structure measuring up to 3 cm in diameter is seen in the right iliac fossa. The lesion is non-compressible, with thickened walls, and shows anechoic to mildly hypoechoic internal content, without septations or internal debris—features suggesting an appendiceal origin.
Color Doppler imaging demonstrates vascular flow along the thickened wall, with no internal signals, indicating preserved wall vascularity without signs of active inflammation.
The linear probe sequence enhances the visualization of wall layering and internal content, confirming the presence of a fluid-filled, encapsulated structure. These findings are consistent with a mucocele of the appendix, a rare condition that may be benign (e.g., retention cyst, mucosal hyperplasia) or neoplastic (e.g., mucinous cystadenoma or cystadenocarcinoma).
Early identification is crucial, as rupture may lead to pseudomyxoma peritonei, a potentially severe complication.
