Martial Arts in Historical Fighting - Or Just Wild Battle Thrashing?

My personal focus is on swordsmanship in a dueling context, based on medieval and renaissance manuals. But it's interesting to speculate about the nature of armed combat on historical battlefields as well.

I say "speculate" because even though there are plenty of sources on warfare in the big picture, like army composition / tactics / / equipment, there isn't much on how the individual warrior or soldier fought, or how they were trained before the Modern era.
