Extreme Ultra Mode No Damage(equals to no death because this mode has no health bar)without Overload, Bonus Pod and 360 Aim(even if I want to use 360 Aim, I can't because I play this game with a keyboard). (Played as Browny). And on Extreme or Extreme Ultra Mode, you can't use perks by default.
I was streaming(not on Twitch or Youtube)while recording this run, and somewhere in level 5, the streaming app made a pop-up(it says there are 52 viewers and I should chat with them but this is very stupid) which made my input have no effect and I had to click my mouse's left button to continue the game. (it scared me but luckily I could just stay still at that point).
