This is the most difficult operation I have ever performed in my entire career as a board certified plastic surgeon. The case of a first-time patient… One who has never before had any type of cosmetic procedure.

A vibrant 61-year-old man was seeking facial rejuvenation for concerns that the visible signs of aging—deepening lines and tissue laxity—no longer reflected his internal youth and vitality.
The biggest obstacle, obviously, was his complete baldness. A traditional facelift, whether a deep plane or high SMAS technique, relies on incisions strategically placed in front of the ear, around the earlobe, and hidden within the temporal and post-auricular hairline. Without hair, these scars would be fully exposed.
For male patients, the goal is an outcome that appears natural and effortless. Visible scars create an unnatural appearance, undermining the subtle rejuvenation. The results must be powerful yet inconspicuous.
So I asked myself: How do I perform a Ponytail Facelift… on a bald man? How do we deliver the significant, transformative rejuvenation expected by the patient without the traditional incisions? Conventional techniques could not meet this patient's unique anatomy. An innovative solution was required.
So we had to brake every rule in the book.
