Examples of Tests: MRIs, CT Scans, Cardiac Stress Tests
Examples of Procedures: Spine or Joint Surgery
Examples of Medications: Humira for Rheumatoid Arthritis, Chemotherapy

Your Doctor or Hospital Ususally Do Not Know What Does or Does Not Require Prior Authorization.

If You Have the Test Without the Prior Authorization, the Insurance Company Will Deny It and the Doctor/Hospital Will Bill You. You Will Then Need to Appeal to the Insurance Company, Which Can Take Months.
