The Direct Anterior Approach (DAA) in hip replacement offers benefits, but like any surgical technique, it has potential complications. Surgeons prioritize minimizing risks, yet DAA presents a higher likelihood of femur fracture if proper soft tissue releases aren't performed or in cases of poor bone quality. This often leads to a preference for alternative approaches in older patients. Nerve injury to the lateral femoral cutaneous nerve (thigh sensation) is a specific concern with DAA. Conversely, the posterior approach carries a risk of sciatic nerve palsy (leg power). Infection is a general risk in hip replacement, underscoring the importance of technique and the surgical environment. DAA requires specialized training due to its steep learning curve.
