Notes:

The new rule for the 100% definition: you must kill enemies, which spawn after a certain condition: so, I'm killing the spiders in the elevator shaft, hands in morgue on "The Sick Ward" level and so on. It doesn't mean that I need to kill spiders of the "Mother Spider" or enemies from the infinite spawners (because of its infinity these enemies have no value). It looks a lot like Max% in Duke Nukem 3D at this point.

Ghost Town: you may miss some kills during the scripted explosion (0:52). It supposed to kill 5 cultists, however, it doesn't sometimes. Save your game before that moment and make sure you get 5 kills.

Ghost Town: due to mapper's mistake, super secret is counted as regular secret as well, resulting in a "10/9" counter.

The Sick Ward: only 47 out of 53 enemies can be killed. These six impossible kills are three glitched stone gargoyles and three rats in the morgue (they don't increase the counter, but the game increases the total kills amount as they spawn along with the hands). Also don't miss two hands in the pram in the secret room.
