On August 22, 2006, Pulkovo Airlines Flight 612 crashed in Ukraine, killing all 170 people on board. The Tupolev Tu-154 encountered severe thunderstorms, but instead of diverting, the crew attempted to climb over the storm to save fuel. Ignoring warnings and operating near the aircraft’s performance limits, the captain pushed the plane to extreme altitude where airspeed dropped dangerously. Despite clear signs of an impending stall, he refused to descend. The aircraft stalled, entered an unrecoverable flat spin, and plunged to the ground. Investigators concluded the disaster was caused by human error, overconfidence, poor crew coordination, and inadequate high-altitude stall training.
