Many prescribers are reluctant to go above FDA guidelines in prescribing stimulants. As with most medicatioins, beneficial effects and side effects tend to increase as dosages of stimulants are increased. But we know that some individuals require higher doses for benefit, and many of them are not bothered by side effects. Informed consent requires patients be aware of the risks of medications they are taking.

Medications should always be prescribed with the response of the individual patient foremost, rather than adhering to recommendations that we know will overshoot and undershoot with substantial fractions of the population.
