Differential Diagnosis for Low Density Hepatic Lesions:
Acute hepatitis
Fulminant hepatic failure eg. Acetaminophen overdose
Disseminated hepatic infection (immunocompromised patients)
Lymphoma
Diffuse metastasis
HCC

MRI is the best test for characterizing liver lesions and should be considered in the setting of:
Known or suspected malignancy
Clarification of findings on other modalities
Abnormal laboratory values
Cirrhotic morphology

Summary:
Hepatic steatosis is very common condition in patients who undergo medical imaging
Being able to recognizing the subtypes and imaging patterns of steatosis can reduce uncertainty and improve patient management
Atypical patterns of steatosis are often related to altered local portal venous perfusion
MRI can non-invasively differentiate fatty change and metastatic disease avoided unnecessary biopsies
