Patients with detrusor instability and stress continence have to take care of themselves. Detrusor instability means when the bladder fills up to some position the bladder over reacts and contracts where they leak urine causing stress leak. In these the patient should be evaluated by doing urodynamics, identifty the cause of over reactive bladder then treat it with bladder relaxants, leading to a lesser contraction of the bladder leading to decrease in the regular pressure causing leakage. Let the patient not keep the bladder full, above a level which causes the bladder to contract causing leak. So these patients have to be treated first knowing the cause. Sometime over activity is present also in cases of urinary tract infection. So if there is an infection, treat the patient, see the over all activity decrease. Otherwise by proving that it is over active bladder by urodynamics treat the patient with bladder relaxants and some amount of mild sphincter relaxants which will cause decrease in stress leak or pain during over activity.
