How to fill out Form W-9

If you've ever worked for someone as an independent contractor, opened a bank or brokerage account, sold real estate, or applied for a mortgage, you've seen Form W-9, which is used by businesses to collected Tax Identification Numbers to either report income (or potential deductions or credits) to the IRS. Despite a recent revision of this form by the IRS, there are still areas of confusion when it comes to filling out this form.
