Why Immune Profiling Is Done?
We usually recommend this report for three situations: multiple failed IVF cycles, recurrent pregnancy loss, or chronic pelvic discomfort. In the case of pelvic pain, we interpret a few other markers too, but for today’s video, we’ll stay focused on standard immune profiling. Our goal is to identify any immune dysfunction in the endometrial environment that could interfere with implantation or early pregnancy.

How the Sample Is Collected?
We perform a simple pipelle biopsy to collect a sample from the endometrium. That sample is transferred into a special medium—manufactured in-house at our hospital—to preserve it for analysis. Once the sample is ready, it is sent to our pathologist for review.
And I must acknowledge our pathologist, Dr. Tat. He has been an essential part of our workflow. Every report we generate is possible because of his expertise and consistent effort.

How to Interpret the Report:
Let’s look at how these markers are scored. If MUM1 exceeds 10 per high-power field, it suggests chronic inflammation—often pointing to endometritis. If BCL-6 is scored above 1.4, it is considered significant and may suggest progesterone resistance or subtle inflammation. A CD56 count higher than 20% indicates an increase in uterine natural killer (uNK) cells, which may interfere with implantation.
But numbers alone don’t tell the whole story. We never interpret these values in isolation. Alongside the report, we review the patient’s ultrasound findings, age, clinical history, and previous embryology records or miscarriage data. A good report interpretation includes all these angles.

How These Results Influence Treatment!
Based on the results, we choose between antibiotic therapy and immune modulation therapy. If chronic endometritis is suspected—often due to high MUM1 levels—we prescribe targeted antibiotics like doxycycline or adjust based on culture reports.

For immune-related findings (high CD56 or elevated BCL-6), we offer treatments like intralipids, tacrolimus, prednisolone, or TNF-alpha inhibitors. The decision is personalized based on severity and history. Not every patient with a borderline report needs medication—context matters.
Important Message for Patients

Do We Combine This With Other Tests?
Yes, quite often. We combine immune profiling with HLA typing, especially when we suspect immunological mismatch between the partners or repeated implantation failure without any clear reason. It helps build a deeper picture of how the maternal immune system is reacting to the embryo.
