Proximal junctional kyphosis is a common complication of adult spinal deformity surgery and has become the greatest challenge in spinal deformity surgery. It occurs in 20% to 40% in most reports.

While some patients with proximal junctional kyphosis have no symptoms, others, called proximal junctional failure patients, show clinical symptoms with pain, gait disturbance, and neurological deficits, and in some severe cases, reoperation may be required. Cases of severe neurological deficits with vertebral subluxation in proximal junctional kyphosis have been reported.
