Interested and eligible candidates may apply online for this vacancy by visiting the official DHSFW Assam Recruitment Portal. Candidates may follow the below-mentioned steps to apply online
1. Go to the Web-Links section.
2. Click on the "Online Application" and complete the registration process by clicking "New Registration".
3. Click on "Online Application" link and log in with your all personal and educational details and upload the necessary documents.
4. Then submit the form.

Note: Don't forget to take a printout of the application form.
For all 4 departments - Directorate of Medical Education (DME), Assam, Directorate of Health Services (DHS), Assam, Directorate of Health Services (Family Welfare), Assam and Directorate of AYUSH, Assam, online applications will be received through a Common Application Form for all Grade III Technical and Non-Technical) and Grade !V posts.

Interested candidates may apply to any post available in the Application Form under any of the Directorates.

Candidates can fill the Application form from any of the websites and need not apply again from other websites.

Candidates must upload following documents along with Common Application Form of DHS, DHSFW, DME & Ayush Departments under Govt. of Assam.

Passport Size Photograph.
Signature of the Applicant.
Age Proof.
Educational Qualification Certificates and Marksheets.
Employment Exchange Registration Number.
Other Technical Qualification Certificates (If applicable).
Caste Certificates (If applicable).
EWS Certificates (If applicable).

Size & Dimension of Photograph and signature

Scan your photograph (maximum 50kb) and your signature (maximum 30kb) and keep ready for uploading during filling up of the Online Application Form.
Photograph and signature must be in .jpg or .jpeg Format.

Photograph:

Size of photograph must be less than 50 kb.
Height of photograph must be between 250 pixel - 320 pixel.
Width of photograph must be between 150 pixel - 220 pixel.

Signature:

Size of signature must be less than 30 kb.
Height of signature must be between 100 pixel - 180 pixel.
Width of signature must be between 150 pixel - 220 pixel.
