Description: Transplanting the crown -- or vertex -- region can be tricky. It requires a significant number of grafts and attention must be paid to the natural "whorl" or twisting pattern in the crown region. But it is an area commonly affected by male pattern hair loss (androgenic alopecia), so patients frequently request hair transplants in this region. This means clinics need to understand how to densely reconstruct the crown area.

The patient in this video suffered from aggressive hair loss. Although he's had a number of procedures performed by our clinic, this presentation focuses on the two procedures he had in the crown region.

Using 3,300 grafts via the gold standard FUT (follicular unit transplant) technique, this patient was able to reconstruct his entire crown.

The procedure began by drawing surgical markings to recreate the appropriate whorl pattern in the crown. The patient was then taken to the procedural suite (which is located within our Great Neck office) and numbed in the donor region. After the numbing in the donor was complete, the patient was completely numb and felt nothing for the rest of the day. After the strip extraction was complete and the donor area meticulously closed with surgical staples, the patient was reclined back and the incision process began.

The incisions are where the new grafts are placed. The incisions determine the angle, direction, and density of the new hairs. In the crown region, this is extremely important and requires great attention and understanding of the crown. While the incisions were created with a custom cut blade (created specifically for the size of this patient's follicles), expert technicians carefully prepared the follicles under high powered microscopes. After the incisions were created and the grafts were prepared, they were placed back into the scalp. The patient was completely awake and comfortable during this process. He was able to relax, rest, use his phone, watch TV, read, eat, and take breaks to stretch and use the restroom whenever he needed.

Because of the progressive nature of hair loss in the crown, this patient underwent this process twice. Once to focus on the upper part of the crown, and another to focus on the lower portion of the crown and the whorl pattern.

He returned 12 months after his last crown procedure, and his results looked excellent. Despite the challenging demands of crown transplantation, his procedure was very successful. He's very pleased with the results, and so are we! So much in fact, that we were able to thoroughly document his results with a "comb-through" video result -- the most honest presentation in hair transplantation.
