Labiaplasty is an operation used to change the shape of the labia minora. We generally grade the labia minora into grades I, II or III depending on whether its less than 2cm, 2-4 cm or more than 4cm from the vaginal introitus. The patients who generally come to see me are grade II or III. The operation I do is the wedge technique. Also known as the butterfly technique.
This is the best technique because it results in one vertical scar, that is located away from the vaginal vault. The alternative technique is the trim. We don’t like doing this as it leaves a scalloped or irregular border along the edge of the labia minora which can cause pain. The trim technique also cuts away all the normal glands that are needed for lubrication. Trimming can often result in an imbalanced look and sometimes the trim means the clitoral hood attachments are left hanging and can leave a deformity which is difficult to fix.
The next decision you need to make is whether you need the hood reduced. This is a hoodectomy. This is an extension from the wedge technique which reduces the clitoral hood from the sides. This gives a more balanced look if you need it done.
About 10% of patients on examination have some webbing at the back of the vaginal introitus. This becomes important if you do the wedge as it pulls that skin forward which can result in rubbing. This can be released at the same time as the labiaplasty.
A note about fat grafting. Some patients want some fat injected into the labia majora. Usually these ladies are older and have atrophic labia majora. The idea is to put enough in, but not too much so as to result in hanging tissue. This can be done at the same time as the a labiaplasty if needed.
Finally a note about labiaplasty and vaginal tightening. Labia plasty is an operation which involves reducing the labia minora. Vaginal tightening is an internal operation. A strip of tissue is removed internally from the vaginal wall and sutured together. As you can see they are very different operations.
