Each part of the Trapezius muscle helps different movements in the body. The upper part of the Trapezius muscle helps to lift the shoulder up, and prevents the shoulder dropping when weight is carried in hand or on the shoulder. The middle part of the Trapezius muscle helps with movement of the shoulder blade back to the center of the body. The lower part of the Trapezius muscle helps with lowering the shoulder, specifically when there is resistance such as a movement as getting up from a chair. The upper and lower part of the Trapezius muscle together help with rotation of the shoulder blade and lifting of the arms above the head.

The Trapezius muscle is used in many movements and sports such rowing, boxing, swimming, sports that involve throwing and more. Overuse of the muscle may lead to injury. Most common injury from overuse of the Trapezius is Trapezius Myalgia. Trapezius Myalgia is felt mainly with pain and tightness at the upper part of the Trapezius muscle. Headaches and stiffness felt at the back of the neck may also indicate an injury involving the Trapezius muscle.

Pain and stiffness in the trapezius is often associated with trigger points (muscle knots). These are extremely common and can be typically addressed with trigger point release techniques by hand, or in some cases with dry needling (medical acupuncture).
