Scholarship Form (can be downloaded from JSMU Website).
Application will be submitted in Students Affairs Department.
The scholarship will be granted under Islamic Principles of Zakat.
Required documents to submit with application:

Istahqaq certificate being issued by Chairmen District Zakat & Usher Committee.
Salary Slip or proof of family income.
Copy of CNIC and Enrollment Card.
Last year / Semester mark sheet.
Copy of Electricity Bill of last month.
