TTTS has a cardiovascular origin. The entire pregnancy needs to be monitored extremely closely for signs the babies are developing heart failure.

Combining the expertise of maternal-fetal medicine specialists and fetal cardiologists, CHOP clinicians developed a unique cardiovascular scoring system to monitor how severe the condition has become for each twin. Fetal echocardiography plays a crucial role in monitoring the pregnancy for subtle changes using the twin-twin transfusion score. By looking specifically at the hearts of both twins, clinicians can look at subtle changes and determine the need for treatment throughout pregnancy. The score is also used as a means, potentially, for prognosis.
