This is a fairly common problem that I've come across. It is only ever found in laminated glass. It normally happens where glass is installed in a frame like a door, window, or balustrade casing.

It is normally due to moisture becoming trapped and being absorbed by the resin causing delamination to occur between the resin and the glass.

The lack of maintenance is often the cause.
