PAPERWORK to FLY with a SERVICE DOG // Filling out my USA DOT Airline Forms

What forms need to be filled out to FLY with a service dog? Every airline has slightly different service dog policies for airplanes. Department of Transportation has service dog guidelines to follow for flights with a service animal. When I first considered flying with Buddy, I didn't know what was required. I had so many questions! But this form is very simple to fill out.
