Played with the Ultra patch from http://cheater.seesaa.net/article/184... . The patch increases bullet quantity and speed and gives the player automatic deathbombs. It's not as insane as some other Touhou modifications titled "Ultra", instead it's roughly what you'd expect from a difficulty mode directly above Lunatic.
