A 20 year old male presented with shortness of breath. Echocardiogram was done, Spot the findings and make a diagnosis on the basis of echocardiography clips shown, answer is given after one minute with explanation of findings.

Case 92: FINDINGS & DIAGNOSIS:
Apical four chamber view showing dilated Right atrium and right ventricle, there is apical displacement of septal leaflet of tricuspid valve and anterior leaflet of TV is sail like. All these findings are consistent wit diagnosis of Ebstein Anomaly. There is also non compaction of LV also evident in apical 2 chamber view. Color doppler and Continous wave doppler showing tricuspid regurgitation.

Role of ECHO in Ebstein Anomaly:
Echocardiography is the imaging study of choice to evaluate Ebstein anomaly. The first echocardiographic criteria to detect Ebstein anomaly is the apical displacement of the septal leaflet of the tricuspid valve greater than 8 mm/m2 in the apical 4-chamber view. The mitral valve annulus is used to calculate the hinge point of the non displaced tricuspid septal leaflet. Right atrial dilatation and interventricular septal flattening due to RV volume overload can also be seen. Tricuspid regurgitation can also be detected with Color Doppler. Echocardiography can help determine the severity of tricuspid regurgitation; however, this may be challenging due to the absence of systolic flow reversal in the hepatic veins (due to equalization of pressures between the right atrium and right ventricle from a large, low-pressure, compliant right atrium) and the inability to calculate vena contracta and proximal isovelocity surface area (due to the possibility of more than one color jet.
