In this video, we have added elder children with hemiplegic spastic cerebral palsy who walk on tip of toes with very sever deformity ant ankle joint: treatment plan includes surgery followed by brace & therapy & case study
Hemiplegic spastic cerebral palsy means affection of one side of body, including upper & lower limb. Mostly, they walk on their toes. Most of the time, they are independent walker and can perform all the functions given to them. Some of the children also have upper limb deformities and functional problems.
In the early stages, these children can be managed with therapy & brace. Sometimes they require plaster. If the tightness is too much, then botulinum toxin injection in the gastrocnemius is needed to relax the muscle. Plaster is also given simultaneously to these children. When they reach 6- 8 years, they develop a fixed length contracture of the calf muscle and require surgery.
But sometimes deformity is very severe, and the child starts walking on the tip of the toes. Then they heel card lengthening the ankle with post joint capsule release, followed by brace and therapy.
