To us it looks like the Project Manager missed updating the Project Management Plan, the project manager shared the details of change request but did not ensure the update in project schedule and work package.

So going forward, it is better to ensure that Approved Change Request get Integrated well in Project Management Plan.

Sharing the details are good but it is not a replacement for updating the project management plan, the project management plan includes a project schedule.

B. Verify the deliverable before taking to the customer

It’s a good idea, but can you think of something even before that, mean how to ensure that something which is not needed does not get build. Better to fix the quality at the source.

C. Updating the project management plan as per approved change request
Can be an option

D. Making team for a frequent meeting with each other
Good idea to ensure that the team remains informed of each other work but not a replacement of update in the project management plan.
