Here is one way to do it - I misspoke in video...I updated Firmware and experienced a problem with the Flash Memory in Line 6 Monkey. Flash eventually updated properly. Here is the Factory Reset at least.

***Update 2021 - Someone in the comments said to have your pedal connected to Line6 Monkey (Software installed on your computer) when doing the reset. I didn't need to until afterwards but others have reported it worked for them once it was connected and they tried the same sequence.

You will have to calibrate your pedal after you do the Factory Reset. This is the best video I've found for that. Couldn't do any better than this guy so, here it is:
