39 y old male patient presented for primary infertility with low volume ejaculate (below 0.5 ml) on several sperm tests, with severe oligoasthenoteratospermia .

Pelvic MRI showed a 2 cm midline prostatic cyst connected to the verumontanum compatible with a prostatic utricle cyst.

The operation consisted on:
incision of the tip of the verumontanum at 6 o'clock
opening of the utricle cyst away from the bladder neck
unroofing of the cyst respecting the opening of the ejaculatory ducts and finally coagulation of the edges of the cyst

A Foley catheter was inserted at the end of the operation for few hours and patient was discharged the same day after removing his Foley catheter.

Patient will be followed with sperm tests in few weeks.
